Basic Concepts of Care Coordination FY19

This class is designed for new Care Coordinators not yet certified. It is a prerequisite to the State of Alaska’s Senior and Disability Services (SDS) three-day Beginning Care Coordination Training. This one-day course is designed to be interactive and provide attendees the opportunity to participate in discussions and activities.

Topics to be covered will include: Ethics, How to Serve Someone with a Disability, Advocacy, Person Centered Planning, and Skills of Interaction.

This training will be delivered as a webinar using Zoom.us. Webinar connection information is available in the AKTC remote-learner portal after your registration and payment are processed.
